CMS requires participating providers to complete an annual D-SNP, or dual special needs plan, Model of Care training. This two-step requirement includes review of course content and attestation of your review.
BCBSRI offers multiple training modalities and sessions for providers to meet these requirements. Providers may:
- Attend a Microsoft Teams-based training offered in Q4 2023; dates are listed below. BCBSRI attests on the providers behalf.
- Independently review the 2024 Mode of Care training content (in PowerPoint) by clicking here and attesting here, via the D-SNP Provider Attestation Form.
- Request a personalized training for your practice by emailing ProviderRelations@bcbsri.org.
As a reminder, training must be completed by the providers themselves (cannot be delegated) by 31 de marzo de 2024.
D-SNP care coordination is unique and designed to help the diverse and complex dual eligible population. We appreciate you helping us to meet this CMS requirement and supporting the D-SNP community.
