Annual D-SNP training due 31 de marzo de 2025

Our Blue RI for Duals (D-SNP) provider network includes all facilities, specialists, our systems of care, and all primary care physicians (PCPs) associated with Accountable Care Organizations (ACOs), Federally Qualitied Health Centers (FQHCs), and Oak Street Health. Any PCP that is not associated with any of those organizations, or specialists not participating with Medicare, will not be participating with this product.

As a reminder, you are required as a participating provider in the D-SNP network to complete the annual D-SNP training by 31 de marzo de 2025. To help providers with completing the training, we are offering multiple sessions for providers to join a live training and we will attest for the provider. This meeting will be held in Microsoft teams and there will be no dial-in as providers will need to review the slides we present. As a reminder, the individual provider needs to take the training and not their office staff.

CMS requires that we train participating providers on our D-SNP model of care, as the care coordination for this plan is unique and designed to help the diverse and complex dual-eligible population. We appreciate you helping us to meet this requirement, and we believe it will get us all off to a great start as we begin to serve and care for these members.

If you would like to take the training and attest on your own, please click here to take the training now. After completing the training, providers are required to fill out the D-SNP Provider Attestation Form to ensure your training is accounted for.
